Dependants visa extension after the main GTV applicant receives ILR

Hi All,

Hope you are doing good and Wish you all a happy new year 2024.

I came to the UK on Feb 2021 via Global Talent visa and my family came to the UK on March’2021. Now with 3 years completion I have got my ILR and I’m working on to extend the dependants visa.

I have multiple questions on the extension part,
which application form to use: Is it the below one

or any other application form? Like ILR dependants family visa application form?

If I use the 1st link mentioned here I have been asked questions on enter the visa end date of the main applicant. As I’m on ILR what date should I enter here to pay the NHS surcharge for my dependants.

Any help would be greatly appreciated.
